Prescribed Medications

After your visit to Occupational Medicine, all prescription medicines prescribed for workers' compensation will need to be filled through our pharmacy network provider, Optum. Complete the Optum First Fill Information [pdf] form to fill your prescription and provide it to the pharmacy when you pick up your prescription.

Reference your Date of Injury on the form in the format mmddyyyy.  A benefit card will be mailed to you after the first fill to use with refills.

This prescription plan is accepted at most major pharmacies.
